Not all data is required. Some data is optional, such as peripheral ultrasound. The patient completes the symptoms part of the form. The practitioner enters the data into the online computer software. A PC or Mac can be used. The software generates a program displayed on the computer screen. The practitioner copies the entire program from the computer screen to a word processor. The practitioner saves the word processor file in the practitioner's computer. No data is stored on our server. The practitioner may make any changes desired in the report. The practitioner saves the report, prints the report, and/or exports the report to a pdf format and email it. The practitioner may also make any changes desired in the report.
